Confidentiality and Your Health Records

The practice is computerised and is registered under the Data Protection Act 1998. Your patient records are kept in paper form and on the computer.We respect your right to privacy and keep all your health information confidential and secure. When you register with us , we create a new health record for you. We also obtain your past health records from your previous GP practice. Patient health records include personal details, details of consultations and other contacts, test results and correspondence.

The Primary Health Care Team (doctors, nurses, health visitors,district nurses, midwives, managers and support staff) use your information to provide healthcare and adminiter the practice. The practice may also share personal information with other people invited in your care, such as hospital doctors.

We also use your information to help us improve the type and quality of service that we have to offer. In addition, we provide information to the NHS for financial and monitoring service.

No information about you is disclosed to anybody outside of the NHS without your consent, unless requested by legal requirement or it is in the public interest. You have the right to see your records. There may be a charge for this service. Ask at reception how to apply. If you think anything in your records is incorrect please let us know.

You have the right to tell us if you prefer that your information is not used in a particular way. Speak to the GP or Practice Nurse if you have any concerns about how your information is used.

Summary Care Record

An electronic summary of your current medication and allergies will be available to emergency GPs and Accident and Emergency departments should you visit them. This information will only be accessed with your consent at the time.Should you not wish this you may can complete an ECS disclaimer form available from reception staff, who will organise for your information to be withheld.

The practice has a publication scheme and will publish information in accordance with that scheme. For more information please ask at reception.
